Dr Kate Haskell, GP Partner, after many years of dedicated service to this surgery has announced her retirement from general practice as from the 29th April 2019.

Dr Haskell will be dearly missed by patients, staff and friends of the surgery.

We would like to wish her a happy and healthy retirement.

We are also delighted to announce that Dr Yasmin Heerah will be joining the team of Partners as from 1st May 2019.

Car Parking Fines: The privately owned dental surgery upstairs have installed cameras on their side of the car park. This is clearly marked as "Dental patients’ car park” above their entrance. To avoid a fine please park on the right side or consider using other local parking facilities.


If you park in the dentist car park

they will send you  a fine of £100


This is being monitored by an external company who initiate the fines, on behalf of the dentists.



Hearing Loop

The surgery has a portable hearing loop for patients hard of hearing with a hearing aid to use. If you would like to use this when you attend the surgery, either in reception or while seeing the Doctor or nurse, please let reception know. You will need to switch your hearing aid to the ‘T’ position.



Repeat Prescription Requests/Online Appointment Booking

We are delighted to be able to offer online appointment bookings.  In addition patients will be able to request their repeat prescriptions electronically.  To register you will need to come into the Surgery with photo identification (driver's licence or passport) and we will arrange for your access to be provided for these services.

Loan of Blood Pressure Machines

Please could all patients ensure that if one of our blood pressure machines is loaned to you for monitoring your blood pressure at home, that it is returned promptly after 1 week and in the condition it was loaned.  There is a waiting list for these machines.

The practice cannot accept registration requests online at present but if you wish you can print out a copy of the :

Questionnaire for new adult patients  (Questionnaire for under 18) and GMS1 Form  bring it into reception along with the following:

(originals must be supplied along with photocopies for our records).  It is the patients responsibility to supply photocopies.

To register at this surgery you must do the following:

  • Live within the practice area – ask at reception
  • Provide proof of identification e.g. driving licence, passport, birth certificate, current visa or identity card
  • Provide proof of address e.g. utility bill, council tax bill, bank statement (dated within the last 3 months)

       Mobile phone invoices will not be accepted

  • Patients under the age of 7 years will need to provide a birth certificate and immunisation history (red book)

If your application is successful, patients under the age of 16 and those 65 and over,  will require a medical with the Health Care Assistant

Complete ALL the forms 

This includes the GMS1 form and the New Patient Questionnaire

Failure to do this will result in an unsuccessful application

Once you have completed all of the above, please hand it back to the receptionist.  The surgery will then handle your application


If your application is unsuccessful you will be contacted and your paperwork will be left at reception for collection
