New Patients

Western Road Medical Centre welcomes new patients who live within our practice area. Please check the tabs on the home screen for a better understanding of the services that we offer. We can also see patients on a temporary residence basis, provided they are staying with one of our permanently-registered patients.

Our medical centre is purpose-built to accommodate the special requirements of those patients who have access needs/are disabled. There is wheelchair access to the consulting rooms, treatment room and disabled toilet facilities. In addition, there are baby-changing facilities. If you have a hidden disability which requires special consideration, please inform a member of staff and efforts will be made to ensure this is provided for.

Private waiting areas are available if required.

Registration

To register at this surgery you must do the following:

  1. Live within the practice area – ask at reception if you need help
  2. Register online using this link. If you have problems registering online please contact reception for further guidance preferably on Tuesdays, Wednesdays, Thursdays and Fridays when reception staff are less busy.

ADDITIONAL

If, subsequently after registration, you are a relative/carer and the patient wishes to consent to us discussing their medical records, please also complete the following:

Carers Consent form

Children under 16 years – parents/guardians will be asked to supply a copy of the child’s immunisation history, red book and birth certificate.

Once received, the registration process may take up to 5 working days, though if immediate treatment is needed, you may be able to request an appointment to be seen on a temporary resident basis.

If the application is successful, any new patients up to the age of 16 years, or patients of any age with long term health conditions should arrange an appointment with a practice nurse for a new patient health check.
